What are the responsibilities and job description for the ID Resource Nurse / Fairgrove Primary Health / Full-Time / M-F 8:30a-5p position at Catawba Valley Medical Center?
Summary of Performance Expectations:
Follows the recommendations given by the physician or advanced practice providers. Follows an evidence-based approach to care. The ID Resource Nurse will provide care coordination, motivational interviewing, consultation and intervention to patients with infectious diseases. Overall duties include supporting clinicians in meeting chronic and preventive health needs of their patients; guiding patients in collaborative self-management for the prevention and management of infectious diseases to include medication adherence. As a part of an interdisciplinary care team, the ID Resource Nurse coordinates patient services such as public health, Disease Intervention Specialist Bridge Counselors, skilled nursing facilities, home health, home infusion services, etc.
Education & Credentials:
Required
- Graduate of accredited nursing program.
- Current NC RN license or licensure from Nursing Compact State.
- BCLS certification. If the BCLS certification is not from the American Heart Association (AHA), an AHA certification is required within three months employment date.
Bachelor of Science in Nursing Degree Requirements:
- Effective May 4, 2015 newly employed Registered Nurses and current employees promoted to Registered Nurse positions are required to obtain a BSN or higher nursing degree. Exception: RNs employed in positions at Catawba Valley Medical Group (CVMG), Fairgrove Primary Health (FPH) and Catawba Valley Infectious Disease (CVID) are exempt from this requirement.
- The BSN or higher nursing degree must be completed within five years from the date of employment or placement into the RN position.
- Incumbents in CVMC RN positions on the effective date will not be required to satisfy the BSN degree requirement to maintain their current position, be promoted to a charge nurse or Operating Room specialty RN position, or apply for lateral RN positions in another CVMC department.
Exceptions:
- RNs transferring to the IRT department are subject to the BSN requirement.
- Effective 03/28/2019: RNs transferring from CVMG, FPH or CVID into RN positions at CVMC are subject to the BSN requirement.
Preferred
BSN or professional certification(s), such as Case Management, Disease Management, Health Coaching, Guided Care Nursing.
Work Experience:
Required
Five years nursing experience. Given training and on-the-job experience, incumbent should be proficient in the basic aspects of the job within three months of employment date.

Care coordination experience using community resources. Experience in coordination of clinical care and/or case management in the acute inpatient and or outpatient setting(s) including home health.
