The Home Visitor Supervisor / Service Coordinator provides supervision and support to the Healthy Families Home Visitors, processes all referrals made to the program including assessment of eligibility, service need and case assignment, assists with data collection, quality assurance tasks and reporting to program funders.
ESSENTIAL FUNCTIONS
- Always maintain professional boundaries and confidentiality.
- Conduct weekly reflective, clinical, and administrative supervision sessions for up to 5 home visitors.
- Promote mindfulness self-regulation strategies to support staff navigate through difficult and stressful work-related situations.
- Facilitate team building activities to promote a culture of belonging, collaboration and staff motivation.
- Train home visitors to guarantee high quality service for the families by meeting the needs of families from diverse socio-economic, cultural, and linguistic backgrounds.
- Maintain up-to-date supervision documentation as required.
- Conduct quality assurance activities such as observations, QA calls, and co-visits to guarantee high quality service for the families.
- Collaborate with the Program Coordinator to analyze data provided by Children’s Trust(CT), develop, and implement strategies to meet all the program required and standards.
- Serve as a linkage between community agencies and staff to provide appropriate referrals and connections for children and families in the program.
- Complete employee performance appraisals.
- As a Mandated Reporter, file and assist home visitors in filing 51A Child Abuse and Neglect reports with the Department of Children, when necessary.
- Oversee health and social service assessment, counseling, education and advocacy services to clients. Make referrals and provide follow-up case management.
- Supervise the participant record and data collection for assigned staff.
- Verify that participants are receiving services by checking weekly employee visitation records and schedules. Ensure data is entered timely and accurately into the Participant Data System.
- Monitor home visitor schedules to ensure home visits are completed according to service level.
- Complete required reports as required by the program and funder.
- Perform annual parent child interaction assessment for program participants utilizing the required parent child interaction rating tool.
- Assist with implementation of HFM policies and execution of expected benchmarks to meet Healthy Families America Best Practice Standards in order to meet national accreditation requirements.
- Supervise home visiting activities, group-based training needs and activities for parents.
- Represent the Healthy Families program to other agencies as requested by the Program Coordinator.
- Maintain ongoing communication with the program funder and community partners.
- Communicate participant concerns to the Program Coordinator and Home Visitor. Resolve timely.
- Familiar with facilitating and supervising support groups or parent education groups.
- Work is performed in an office environment and at offsite locations as required. Use of general office equipment including phones and computers. The Supervisor will support the Home Visitor with home visitation during the absence of the Home Visitor.
- May stand or sit for prolonged periods of time.
- Open or close the office as required. Always ensure safety for self and others.
- Attend internal and external meetings and training courses as required.
- Local travel as required.
- Ability to work early mornings, evenings and weekends as needed.
- Ability to navigate stairs in residences without elevator access.
- Other duties assigned by the Program Coordinator.
