- Salary $42,500
- Works as part of a multi-disciplinary team grounded in the housing first approach and coupled with an integrated service model to assist families in obtaining and retaining housing in Somerset, Hunterdon and Warren Counties.
- Responsible for establishing a trusting relationship with families and working with them to identify their service goals.
- Holistic planning approach to assist families with connecting to, navigating and coordinating needed health and social services.
- Rotating 24/7 coverage required with Case management and case conference with CP&P and other providers.
- Bachelor’s Degree in related field required with 2 years' experience.
- Remote work possible after 3-month probationary period.
- Driver’s license needed with an acceptable driving history.
