PACT Wellness/Housing Specialist Program 068
$42,000 with Sign On Bonus $3,000
Responsibilities
Works as part of a multi-disciplinary PACT team
Provides community-based treatment and skills training to persons with serious mental illness
Responsible for identifying, locating, and securing housing for individuals with mental illness
Works closely with state psychiatric hospitals to facilitate discharges of individuals into the community.
Requirements
AA/BA/BS/BSW
Minimum of 2 years working with mentally ill population OR a Master’s degree and 1 year post-bachelor’s experience
Experience in housing options for difficult to place individuals preferred
Valid driver’s license with an acceptable driving history
