What are the responsibilities and job description for the Partial Care Counselor (Hourly) position at Catholic Charities - Diocese of Trenton?
JOB SUMMARY: Under the supervision of the Program Director and/or Supervisor, the Partial Care Counselor serves as a member of the treatment team for consumers of the Partial Care Program. Works as part of a team providing group and rehabilitation counseling and service coordination to promote recovery of consumers.
RESPONSIBILITIES:
- Display commitment to the goals of Delaware House Partial Care program and its responsiveness to the needs of consumers.
- Facilitate groups and activities that address the social, vocational, financial, and skill learning needs of the consumers.
- Participate in treatment planning and review representing the needs and goals of the consumer as presented during Partial Care Services.
- Maintain accurate and current documentation of the consumer's participation and services provided in the Partial Care Program.
- Participate in staff meetings and training opportunities.
- Perform other duties as may be deemed necessary and appropriate by the Program Supervisor and Director.
