For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.
NORC Sheepshead Bay Program provides supportive case management and health-related services to the seniors residing in NYCHA housing development in Sheepshead Bay - Nostrand communities. The program assists seniors in healthy aging within the naturally occurring retirement community. The program offers on-site nurses, blood pressure screenings, and medication reviews. Social Services available through the program include case management, advocacy, entitlement assistance, referrals, Recreational / Educational lectures, and activities. The program offers Volunteer Opportunities which include escorting seniors to the appointments. Telephone reassurance and visit homebound seniors are part of the program as well.
STATEMENT OF THE JOB
The Case Manager II provides comprehensive services to senior tenants residing in the Nostrand and Sheepshead Bay Housing complex through support services to maintain the client at home with a safe care plan. This staff will work with local Older Adult Centers to ensure continuity of care and implement an "aging in place" strategy for seniors in the area.
- Provide a comprehensive in-home assessment and reassessment of the older person's current functional health and cognitive capacities and limitations, unmet needs, and eligibility requirements for benefits and entitlements.
- Develop and implement a care plan with the senior and his / her family when needed to address the client's unmet needs and assist them in continuing to live safely in their own homes.
- Provide crisis intervention services when needed for situations such as medical emergencies, utility shut-offs, bedbug infestations, and disaster response.
- Perform intake activities, which include assessing eligibility, preliminary evaluation of the presenting problem, initiating services as needed, and providing information and referral services.
- Maintain up-to-date case notes and the timely submission of quarterly statistics and reports as required by the agency and the funding source.
- Utilize the PeerPlace data system for client information and screen for benefits.
