JOB SUMMARY : The Case Manager Supervisor's goal is to improve the well-being of clients by supervising the delivery of case management services including the coordination of community resources. As a Case Manager Supervisor, the position supervises the case management operations of the assigned area, ensuring practices are in accordance with best-practice standards of the profession, contract / grant requirements, and Agency practices and procedures. The Case Manager Supervisor provides input on the program operations, program development, staff training, performance management, and personnel activities.

Supervisory Responsibility : Provides administrative and functional supervision to 5 full-time employees.

ESSENTIAL DUTIES and RESPONSIBILITIES :

Administrative

  • Provide case management supervision, case consultation, and incident management for all direct service staff, interns, and volunteers.
  • Facilitate regularly scheduled case conferences and skill-building meetings for the Case Management staff to build competencies in best department protocols.
  • Ensure compliance with contracts, agency policies and procedures, accreditation requirements, ethics, best practices, and quality standards within the field.
  • Collaborate with and provide necessary referrals to other disciplines including legal, medical, dental, recreational, educational, vocational, substance abuse and mental health professionals, and other housing or service providers.
  • Assist with the hiring and supervision of Case Management staff and student interns as needed.
  • Meet individually with Case Managers to plan and review cases, discuss engagement strategies, and evaluate the effectiveness of case managers and their services.
  • Organize and participate in client file reviews (program and agency individual service review process).
  • Implement corrective action plan to address deficiencies identified through the individual service review.
  • Create and maintain case records, including, but not limited to legal documentation, psychosocial, case notes, case plans, service updates, court reports, medical reports, UIRs (Unusual Incident Report), referrals, discharge summaries, visitation records, and educational information.
  • Facilitate random client satisfaction surveys for quality assurance and staff supervision.
  • Conduct performance evaluation of case management staff.
  • Model professional behavior and positive attitude.
  • Complete reports on time and maintain statistical data on an ongoing basis.
  • May share joint coaching and staff disciplinary action with another supervisor and / or Sr. Program Manager.
  • Review, evaluate, and approve participant assessment and case plans for quality and effectiveness
  • May carry a caseload, depending on the needs of the program.
  • Ensure the improvement of client services' outcomes and customer satisfaction by reviewing clients' charts and that all surveys are completed by the deadline.
  • Oversee quality improvement efforts for case management staff and enable staff participation in program and agency quality improvement.
  • Oversee team and program outcome measures and data tracking for quality improvement and supporting Sr. Program Manager with quality assurance initiatives.
  • Perform all other job-related duties as assigned.
  • EDUCATION and EXPERIENCE :

  • A Bachelor's Degree in the Human Services field with 4 years of work experience providing direct service to vulnerable populations.
  • Two (2) years of progressively responsible 1-on-1 and team intern / case management supervision.
  • Driving is required. Must have a valid driver's license and clean driving record.
  • Driving in the community to clients' homes is expected occasionally.
  • SKILLS and COMPETENCIES :

  • Ability to communicate effectively in written and oral. Ability to respond to and work effectively with stakeholders.
  • Ability to work effectively with clients in a diverse community. Ability to supervise and train employees and foster a cooperative work environment.
  • Skill in the use of computers, preferably in a PC, Windows-based operating environment.
