As the Care Coordination Services, you will play a pivotal role in individuals' lives, making a significant impact through:
- Make initial contact with eligible participants within 3 hours of receiving referral
- Facilitate initial intake with referrals
- Deliver outreach and engagement services in accordance with the service plan
- According to program guidelines, consult with case management staff, Health HUB staff and/or participant’s providers on progress of service/care plan services
- Conduct prevention related harm reduction/risk reduction interventions with participants
- Provide linkage and navigation to needed services for family members and other identified support systems
- Maintain case records and documentation in accordance with agency and OASAS standards
Care Coordination Services Minimum Salary: $ 19.50
Care Coordination Services Maximum Salary: $ 20.51
Care Coordination Services Schedule: Mon - Fri (35 Hours)
Qualifications:
- High School Diploma or GED
- 1-3 years of experience
- Valid driver’s license, proof of current car insurance and registration and access to a reliable, working vehicle
