As the TBI Service Coordinator, you will play a pivotal role in individuals' lives, making a significant impact through:
- Providing service coordination to individuals and families residing in the community at large
- Maintaining current knowledge base of DOH policies, procedures and regulations related to service coordination
- In general, the services entail helping individuals and families access appropriate programs, benefits and other opportunities that improve their quality of life
TBI Service Coordinator Minimum Salary: $20.51/hour
TBI Service Coordinator Maximum Salary: $23.07/hour
TBI Service Coordinator Schedule: M-F 8:30am-5:00pm; Combination of in-office and remote work
Qualifications:
- Valid NYS Driver’s License

Experience is required. You must:
- (A) Be an individual with a Master’s degree/PHD in Social Work, Psychology, or related field. Or have one of the listed professional licenses:
- (B) Be an individual with a Bachelor’s degree and three (1) years’ experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or Traumatic Brain Injury; OR
- (C) Be an individual with an Associate’s degree and five (2) years’ experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or Traumatic Brain Injury
