Social Work Case Manager – St. Joseph House of Hospitality
Catholic Charities offers practical assistance to those who are experiencing homelessness or at-risk of losing their homes. Founded in 1937, St. Joseph House of Hospitality is the only year-round men's residential program of its kind in Southwestern Pennsylvania. To be eligible, candidates must be experiencing homelessness or at risk of homelessness, must have a limited income, and be fifty years of age or older.
We’re seeking a compassionate Case Manager to ensure that our residents receive comfort, support, and a sense of belonging. The Case Manager will provide intensive case management support to assist our residents with effectively addressing the issues that have contributed to their homelessness.

You’re in a key position to facilitate much-needed relief for members of a vulnerable population. You have fresh ideas to set people up for success and navigate life’s challenges.

Key Performance Objectives Include:
- Works collaboratively with other agencies for referrals regarding needs for homeless individuals.
- Schedules the initial assessment and intake screening of all clients applying for housing; orients residents by explaining program rules and expectations.
- Provides individual and group case management services; coordinates group meetings such as program participant meetings, support groups;
- Assists residents with the development of a service plan; monitors individual service plans; addresses identified problems and implements monthly reviews, measurable goals and action plans necessary for successful independent living.
- Prepares and maintains up to date confidential and organized client files including comprehensive assessments, service plans, referrals, intakes, termination reports, progress notes and other records as necessary.
- Tracks and ensures that residents assigned to transitional beds are meeting benchmarks to transition to their own home.
- Represents Catholic Charities’ Mission and Vision in the community; attends local and divisional meetings pertinent to homeless services.
Minimum Qualifications:
- A bachelor’s degree in social work or a human services field or an equivalent field is required.
- A minimum of two years of relevant social work experience is required. Internship experiences are acceptable.
- Microsoft Office skills and documentation skills are required.
- Previous experience working with individuals and groups is required.
- A demonstrated record of excellent oral and written communication skills is required.
- Knowledge of community services and resources is required.
- A strong desire to work with the homeless population is required.
- A current and valid driver’s license is required.
- Must be available to provide case management to residents two evenings per week.
