University of Pittsburgh Physicians Internal Medicine Recovery Engagement Program (IM REP) is hiring a full time Social Work Case Manager to join their team in Pittsburgh, PA! In this position you will work Monday-Friday daylight hours supporting individuals experiencing addiction. While previous experience with this population is not required, it is preferred.

Purpose:
Provide patients with support and assistance in accessing resources and programs. Assist in improving the overall physical and mental health of patients by coordinating care. Collaborating with behavioral health and medical providers to identify complex cases.
Responsibilities:
- Evaluate and initiate appropriate referrals for assistance through County, State, and Federal programs.
- Assisting patients with connections to community resources in the life domains: (1) housing (2) legal (3) food (4) utilities or (5) health care services
- Assist patients in completing application for free medication programs.
- Provide information and education about health insurance, medical assistance, Medicare, and UPMC financial assistance.
- Complete social work consults with health center patients from birth to geriatric.
- Case management of health center patients to ensure that connection with the correct provider and follow up.
- Working with patients to complete necessary forms
- Oversee Center of Excellence quality and data management
Salary : $24 - $39
