Job Overview
Catholic Charities Steuben/Livingston is seeking a skilled Crisis Intervention Specialist to provide intensive, in-home crisis intervention services to youth at risk and their families. Key Responsibilities
- Carry a low caseload of up to 3 families for approximately 4-6 weeks for crisis services, conducting multiple visits weekly and providing crisis intervention in the client's home environment.
- Conduct screening, assessment, and planning for youth/family in accordance with program standards, collaborating with support teams to create individualized safety plans.
- Develop evidenced-based, individualized treatment plans in collaboration with youth/family, providing de-escalation and crisis support as needed.
- Collaborate with community agencies, coordinating services to facilitate crisis management, family rehabilitation, reunification, and child permanency.
- Provide outreach, family support and education, and referral linkage to community resources, maintaining timely record keeping and case documentation in the electronic health record.
- Participate in reporting and statistical information as required, attending weekly individual or group supervision and staff meetings as scheduled.
Requirements
- Licensure as a New York State Qualified Health Professional (LMSW or LMHC) or Master's degree in related field with relevant experience in community-based mental health or case management.
- Bachelor's degree with relevant experience in community-based mental health or case management, with additional requirements including confidentiality, verbal and written communication skills, ability to multi-task and prioritize duties.
- Valid NYS Driver's license and reliable, registered, and insured vehicle, with commitment to Agency Mission Statement and quality care principles.
Benefits
- Salary: $22.00-27.00/hour depending on years of experience/education level.
