- Provide direct patient care, administering medications, changing dressings, monitoring vital signs, and charting with timely documentation
- Collaborate with a variety of healthcare professionals, including social work, care coordination, physical therapy, occupational therapy, lab, respiratory therapy, and imaging
- Develop and implement individualized care plans, taking into account patients' unique needs and preferences
