Nurse Practitioner - Primary Care and Urgent Care
The Nurse Practitioner is an integral part of the delivery of the health care delivery team and provides for primary care in many ambulatory settings through the CHS Physician Enterprise. The NP is privileged and credentialed through the Medical Staff process. Duties include, but are not limited to: obtaining a detailed patient history, performing a comprehensive physical examination, diagnosing medical problems, ordering and interpreting diagnostic studies, instituting appropriate medical treatment, ordering medications, performing invasive procedures, education and counseling of patients. The NP should be able to perform an all the duties and responsibilities as described in the PA, RN, LPN, and MA Job descriptions.
Duties and Responsibilities
- Collaborates with the physician and other providers in the provision of patient care.
- Follows established policies for treatments and procedures.
- Greets and welcomes patients. Uses two (2) identifiers for patient identification (name and date of birth). Completes detailed history and physical examination on patients.
- Assess patients for basic vital signs, chief complaints and purpose of visit and reviews charts for recent visits, chronic problems, recent and outstanding laboratory studies, and other information that would facilitate prompt and complete care.
- Provides telephone triage for patients requesting information or advice and assisting with and encouraging appointments when appropriate.
- Obtains additional data from patient/significant other, EKG/lab/ diagnostic test results and other historical data.
- Evaluates patient responses in relation to established goals and contributes to the revision of the plan of care.
- Writes appropriate orders, prescribes/renews prescriptions and provides patient education.
- Reviews laboratory findings and diagnostic testing for follow up, addressing critical findings in a timely manner.
- Assists with creating provider and practice level quality performance reports.
- Coordinates a wide range of self-management support and education to foster patient engagement and activation.
- Delivers interventions based on standard orders and established written protocols to improve the health of the patient population.
- Works collaboratively with members of the healthcare team to develop tools and interventions designed to achieve optimal patient outcomes.
- Prepares, administers and documents medications and vaccines.
- Collects, prepares and processes laboratory specimens for on-site and remote-site testing. Tracks and enters results in medical record. Performs quality control for waived testing and log maintenance.
- Performs, prepares for and assists with in-office procedures and diagnostic testing.
- Assists with the scheduling of procedures, referrals and specialty services and testing as needed.
- Maintains and uses principles of aseptic techniques and infection control when performing clinical duties and tasks.
- Demonstrates ability to critique own practice and accepts constructive feedback regarding performance. Pursues corrective action.
- Participates in team meetings/huddles to foster communication and provide feedback to providers, patients, families, pharmacies, vendors, etc.
- Aids in the orientation of new staff. May precept student clinicians.
- Participates in Hedis, GPRO, and other quality programs as needed
- Enters provider orders into the electronic record for provider review and signature
- Preforms AWV
- Patient counseling and instruction on health maintenance goals
- Works with healthcare team to assure smooth and efficient patient flow and clinical operation.
- Works to create and maintain excellence in patient/family satisfaction, relationships and engagement. Responds to care questions or complaints for timely resolve. Provides documentation on unresolved issues voiced by patients and refers to appropriate party for resolution.
- Operates clinical equipment according to manufacturer’s instructions including safety and quality control checks. Checks care areas for malfunctioning equipment and general maintenance problems on a regular basis. Reports findings to appropriate person or arranges for repairs, as required.
- Ensures that blood/body fluids, biohazardous materials, needle containers and potentially infectious waste are appropriately disposed and removed from the clinical area according to infection control guidelines.
- Other related duties, as assigned.

Position Requirements and Qualifications
Education:
- Degree from an accredited Nurse Practitioner program
- Current NYS RN and Current NYS Nurse Practitioner license FNP
- Current national certification
- Maintains continuing education and log as required for certification and/or licensure.
- BCLS training and current certification required from: American Heart Association ACLS current certification.

- Minimum of two years’ experience as a Nurse Practitioner in a physician practice or ambulatory environment or previous related experience.
- Responsible for personal continuing education and development needs as required. Participates in professional meetings, educational course, or other training that improves knowledge and skills needed in fulfilling the duties of the position.
Salary Range
USD $66.55 - USD $90.00 /Hr.
