Full-Time Hours: Monday-Friday 8AM to 5PM


Summary & Objective

The Counselor, Bereavement is a qualified counselor who is responsible for assessing and providing emotional, psychosocial, and spiritual support and services provided before and after the death of the patient to assist with issues related to grief, loss, and adjusting.


Essential Functions

  • Provides bereavement services to include home visits and telephone correspondence, in conjunction with the Interdisciplinary Team or as requested by the family or significant others.
  • Assesses the social and emotional factors related to family members/survivors/primary caregiver coping with a loss.
  • Provide accurate and appropriate documentation and submit daily.
  • Provides appropriate bereavement counseling services to reflect the needs of the bereaved.
  • Develop a bereavement plan of care that notes the kind of bereavement services to be offered and the frequency of service delivery.
  • Participates in clinical supervision of students as related to bereavement issues.
  • Provide Individual counseling as needed by family members/significant others.
  • Participates in community outreach programs such as but not limited to: ADEC, Dade County Human Services Coalition etc., and is available as a resource for these programs
  • Maintains clinical records including but not limited to bereavement contacts with survivors.

Other Duties

  • Provides input as requested to the Team Manager regarding policies and procedures.
  • Facilitates Bereavement Support Group.
  • Educates community in the needs of the bereaved.
  • Participate with on-call rotations.
  • Handle sensitive information, maintains confidentiality, and coordinates several projects at the same time.
  • Demonstrates knowledge of hospice levels of care, Medicare and Medicaid Hospice Benefits by providing appropriate documentation when the need arises.
  • Consistently able to work calmly and maintain effectiveness in situations of high stress.
  • Maintain your required licenses, certifications and mandatory skill updates.
  • Comply with all policies, local, state and federal laws and regulations.
  • Provide other duties of healthcare team member
  • Perform other duties as assigned

Supervisory Responsibility

  • May serve as an interim department leader depending on need

Physical Requirements

  • Must be able to lift and/or move up to 50 pounds and push/pull up to 250 pounds, walk, climb stair or ladders, stand on feet for extended periods of time, etc.

Requirements

Knowledge & Experience Requirements

  • Master’s Degree in Mental Health, Psychology, Social Work or related field.
  • License in Social Work, Mental Health or Marriage and Family Therapy preferred.
  • Minimum of 2 years’ experience in social work or counseling position with an emphasis on bereavement preferred.
  • Experience working for a foundation or non-profit organization is preferred.
  • Must have knowledge of computer office/clinical software.
  • Must be able to read, write and understand the English language.
  • Must be able to provide own transportation to various locations in service areas as required by duties.
