The Case Manager assists with meeting client goals. Provides case management activities, as well hands-on assistance to complete tasks; provides crisis intervention, and other supports as needed. This position assist seniors, age 60 residing in Franklin County, to help them remain independent in the community. Provides case management activities, assisting with application and maintenance of benefits, medical advocacy, financial management, housing resources and community service linkage. Provides emotional support and crisis intervention as needed.

Key Responsibilities
- Coordinates with Program Coordinator in developing and carrying out service plan goals.
- Maintain and balance a caseload of 20-30 clients.
- Meet annual productivity expectation of 1074 hours or approximately 89.5 hours monthly.
- Complete home visits and telephone calls to assist with case management functions. Assist in accessing community resources, including making referrals and/or completion of applications or other required documentation.
- Accompanies clients with medical appointments; communicates with medical personnel around client needs as assigned.
- Assists clients in maintaining necessary benefits and health insurances include coordinating appointments, completion or obtaining required documentation and/or attending appointments.
- Must be able to work independently.
- Enter all case notes within two days of client interaction.
- Provides evidence of referrals given or made to community support, resources, and other services providers in case notes service fields within two business days.
- Represents the agency with the community and follows office procedures.
Experience
- Associate’s degree in human services, education or related field, bachelor’s degree, preferred.
- Two (2) years of case management service experience with diverse, low-income populations, or combination of education and experience.
- Knowledge of community resources and entitlement programs.
