The Admission Clinical Liaison plays a crucial role in the success of Cavalier Healthcare by serving as a point of contact between the facility and prospective residents, their families, and healthcare professionals. This role is responsible for facilitating the admission process, conducting clinical assessments, and ensuring that all necessary information and documentation are complete and accurate.
Key Responsibilities:
1. Admission Assessment:
- Conduct comprehensive clinical assessments of prospective residents to evaluate their medical needs, functional abilities, and special requirements.
- Collaborate with healthcare professionals, including physicians, nurses, and therapists, to gather relevant information.
- Evaluate the compatibility of the facility’s capabilities with the prospective resident’s care needs.
- Review clinical paperwork and make admission decisions timely.
2. Insurance and Financial Verification:
- Obtain and verify insurance information from prospective residents and healthcare providers ensuring accuracy, coverage details, and completeness.
- Contact insurance companies or third-party payers to verify eligibility and benefits including preauthorization requirements.
- Initiate Pre-authorizations with insurance companies including providing all necessary clinical documentation.
- Ensure all required forms and documentation are completed accurately and submitted within specified deadlines and timeframes.
- Provide residents and families information regarding long-term care Medicaid and other financial resources.
3. Documentation and Compliance:
- Ensure that all admission documentation is complete, accurate, and in compliance with regulatory guidelines.
- Assist in obtaining required consents, advanced directives, insurance information, and any other necessary documents.
- Entered Resident information in software such as PointClickCare.
4. Communication and Consultation:
- Establish immediate communication with referral sources through portals and traditional communication avenues to ensure timely and accurate communication.
- Establish immediate communication internally with the facility and clinical team to ensure smooth discharge into the facility.
- Serve as point of contact for inquiries related to admissions.
- Offer guidance and support to families during the decision-making process, addressing their concerns and questions.
- Partner and communicate on a regular, daily basis with the external Marketing Liaison.
- Provide internal marketing support through facility tours and assist external Marketing Liaison as needed.
5. Transition Planning:
- Coordinate the transition/admission process for new residents, including arranging transportation, room assignment, and necessary equipment.
Other:
Any other duties and responsibilities requested by the Owner/Corporate team.
Present with a professional appearance and attitude.
Promote positive relationships with residents, visitors, and government regulators.
Attend and provide in-service as indicated.
Prevent abuse, neglect, and exploitation in the elderly.
Become a participating facility team member with the residents as a priority.
Attend facility meetings as designated by the Administrator.
Qualifications:
Registered Nurse(RN) or Licensed Practical Nurse(LPN) with a valid state license.
Prior experience in healthcare admission, discharge planning, or case management is preferred.
Strong clinical assessment skills and knowledge of relevant medical terminology.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Proficiency in computer applications and electronic health records (HER) systems such as POINTCLICKCARE.
Understanding Medicare, Medicaid, and private insurance regulations is a plus.

OTHER CONSIDERATIONS AND REQUIREMENTS:
Must be able to tolerate a high pace as typical for a nursing facility. Must be able to take and give instructions well and enjoy working with the elderly. Although the employee is required to sit for prolonged periods of time, he/she must also be able to tolerate prolonged standing and walking.
