Clinical Intake Specialist

Working under the general supervision of the Executive Director, the Clinical Services Intake Specialist schedules new patient admissions and referrals. Understands and applies both state and federal home care regulations.


Roles and Responsibilities:

  • Schedules new patient admissions to home care services.
  • Communicates appropriate information to clinicians regarding new admissions and referrals; provides clinicians with appropriate documentation. This includes complete SOC packet for SOC nurses and documentation being attached to the chart.
  • Ensures that patients are called and screened for services the same day as referral is received.
  • Ensures that needed orders are obtained to initiate services.
  • Ensures that referral sources are kept up to date on status of referral and any delays in SOC.
  • Strives to meet State and Federal guidelines of initiating services within two days of receipt of referral.
  • Communicates with other members of the clinical team
  • Updates Executive Director and the DPS on the flow of assignments and other issues.
  • Attends daily PM Intake coordination call and has master sheet prepared for the call with referrals identified that are appropriate to be started the next day.
  • Work with Clinical Team Coordinator and Clinical Team Leaders in creating a smooth transition from intake to the clinical team.

Required Skills and Experience:

  • The successful incumbent must possess a Licensed Practical Nurse Certification from an accredited nursing program and have current licensure in New York State.

Preferred Skills and Experience:

  • Preference will be given to candidates possessing scheduling experience, as well as those who have experience with the Agency's medical data base system and Microsoft Office Software. The following is also required:
  • Must be able to handle stress in order to meet intake needs.
  • Must have excellent communication and diplomacy skills.
  • Proven excellent customer service skills in order to establish excellent rapport with referral sources, community partners and practitioners.
  • Must be a skilled organizer ability to pay attention to details, ability to problem-solve and multi-task.
  • High degree of confidentiality required.

Physical Requirements:

  • Preference will be given to candidates possessing scheduling experience, as well as those who have experience with the Agency's medical data base system and Microsoft Office Software. The following is also required:

Location and Travel Requirements:

  • This position is located in a pleasant office environment, and will receive a moderate level of public contact. The incumbent will interact regularly with Clinical Leadership team, Case Managers and Therapists.

Pay Range Disclosure:

  • $24.04 to $32.21 per hour
