Job Summary:
•Maintain relationships with referral sources. Send out information on all services offered and market to the public by giving tours and hosting groups from the community.
•Screen referrals in conjunction with Nursing, including review of medical charts and other incoming data from referral sources to determine matched bed availability based on diagnosis/prognosis and ability to meet determined needs. Review prospective resident medication list with a subsequent cost analysis.
•Determine resident payer source(s) and insurance coverage(s), and obtain pre-authorizations and pre-certifications as required by individual insurance carriers.
•Greet new admissions and facilitate smooth transitions and ensuring all necessary signatures from resident/responsible party at the time of admission.
•Ensure in-house transfers are coordinated with Nursing, Social Services Department, Medical Records, and Housekeeping.
•Explain discharge procedure when applicable.
Requirements:
•Prior admissions experience in healthcare setting required, preferably in long-term care.
•Knowledge of insurance authorization processes and medical terminology.
•Strong communication and interpersonal skills.
•A criminal background check and physical will be conducted prior to hire date.
