Job Summary: The CDPAP Coordinator position will assist Consumers in the Consumer Directed Personal Assistance Program at CCOR.
Responsibilities:
- Train and provide on-going support to Consumers on the operations of CDPAP.
- Educate Consumers on Compliance, and Medicaid Fraud Prevention.
- Process Attendants Time Records while following Electronic Visit Verification standards.
- Facilitate quarterly meetings with consumers.
- Maintain paperwork required by the Department of Health for CDPAP, including but not limited to time records and authorizations and Medicaid eligibility.
- Adhere to all policies and procedures related to CDPAP.
- Uphold the companys core values.
- Perform other duties as assigned.
Pay: $19 to $21 per hour
Responsible To:
- CDPAP Coordinator Supervisor
Position Requirements:
- High School Diploma required.
- Associate degree in Human Services, Social Work, or related field preferred.
- Bilingual preferred.
- Ability to work remotely and also to report to work in the office, in a hybrid capacity.
- 2 years experience working with older adults or individuals with disabilities.
- Excellent customer service skills.
- Excellent verbal and written communication skills.
- Organizational and time management abilities.
- Ability to relate to diverse and multi-cultural populations.
- Completion of a background check.
- Appearance is professional and complies with agency dress code.
- Ability to lift up to 25 pounds and to sit, stand, bend, stop, kneel, pull, climb, reach, and perform repetitive moments of the upper extremities.
- Pass a physical examination and immunization requirements after being offered employment and complete health assessment annually, thereafter, including the first shot of COVID-19 Vaccination by October 7, 2021, per NYS regulations.
- Possess a current, valid New York State drivers license and a reliable vehicle.
- Completion of CCORs orientation program.
