Cedar Hills Hospital is seeking a Discharge Planner with an background in behavioral health to join our team and support patients!
How you will make an impact :
The Discharge Planner advocates for patient access to services during treatment team meetings, helping to support and coordinate ongoing patient care and aftercare needs. Additional responsibilities include :
- Promoting effective use of resources for patients
- Advocating that the patient is placed in the appropriate level of care and program
- Maintaining documentation related to case management activities
- Oversight of continued communication of treatment progress, discharge plans, and time of discharge to outside parties
- Facilitating the transfer of required documents to the outside agents

What We Offer :Pay Range : $27.00 to $31.00 per hour
