DISCHARGE PLANNER

Cedar Hills Hospital is seeking a Discharge Planner with an background in behavioral health to join our team and support patients!

How you will make an impact:

The Discharge Planner advocates for patient access to services during treatment team meetings, helping to support and coordinate ongoing patient care and aftercare needs. Additional responsibilities include:

• Promoting effective use of resources for patients
• Advocating that the patient is placed in the appropriate level of care and program
• Maintaining documentation related to case management activities
• Oversight of continued communication of treatment progress, discharge plans, and time of discharge to outside parties
• Facilitating the transfer of required documents to the outside agents

Pay Range: $27.00 to $31.00 per hour

The range displayed on each job posting reflects the base rate of pay and minimum and maximum target for new hire salaries for the position. Within the range, individual pay is determined by factors including job-related skills, experience, and relevant education or training. • Bachelors from an accredited college or university in social work, mental health, psychology, nursing or related field preferred.
• Experience in a healthcare setting or managed care
• Patient assessment related to discharge planning, treatment planning, and/or case management experience preferred
• May be required to work occasional overtime and flexible hours
