- Collaborate with physicians, case managers, and discharge planners to ensure smooth transitions for patients entering the facility.
- Develop and maintain strong relationships with referral sources, including hospitals, rehabilitation centers, and other healthcare professionals.
- Coordinate with the Admissions Coordinator and clinical staff to prepare for new patient admissions, ensuring appropriate care plans are in place.
- Educate patients and their families about the benefits of Cedar Ridge Health and Rehab's services and provide clear communication about the admission process.
- Act as a liaison between healthcare providers and the facility, ensuring patient information is accurately communicated and documented.
- Represent Cedar Ridge Health and Rehab at hospital and community events, furthering relationships with potential referral sources.
- Work closely with the facility's clinical leadership team (DON, ADON, MDS) to review and track patient care outcomes and ensure a high standard of care is met.
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) certification required.
- Previous experience in a Clinical Liaison or similar healthcare position preferred.
- Strong understanding of clinical criteria and the skilled nursing admissions process.
- Excellent communication skills, with the ability to engage diverse healthcare providers and staff.
- Ability to assess patient conditions and provide appropriate recommendations for care.
- Professional, dedicated, and knowledgeable about skilled nursing services and healthcare regulations.
