Job Summary:
The Designated Coordinator is responsible for the development of individual service plans based on a thorough assessment of persons served interests and abilities; coordinating this plan with other staff and outside support team members. Caseload may be comprised of waiver participants, Extended Employment persons served and/or participants in seniors programs.
Essential Job Responsibilities:
- Responsible for all case management duties including person-centered planning, assessments, service plans, progress reviews, data collection, and documentation.
- Provide direction and support for direct service staff both in the facility and in the community.
- Assure quality programming/service, which meets waiver licensing, CARF requirements and other applicable regulations.
- Participate in program development meetings, assists with special projects and activities, etc.
- Represent Cedar Valley in a professional manner with stakeholders and outside agencies, providing effective public relations.
- Transport persons served to meetings, job sites, etc. as needed.
- Participate in on-going self-development through meetings and training opportunities.
- Other duties as assigned.
Educational Qualifications and Work Experience:
Bachelor’s degree in Human Services related field and one year of full-time work experience providing direct care services to persons with disabilities; OR A.A. Degree in Human Services related field and two years of full-time work experience providing direct care services to persons with disabilities; OR Diploma in Human Services related field from an accredited postsecondary institution and three years of full-time experience providing direct care services to persons with disabilities; OR Minimum of 50 hours of education and training related to human services and disabilities; AND Four years of full-time work experience providing direct care services to persons with disabilities under the supervision of a staff person who meets one of the first three qualifications.
Position requires valid driver’s license, current vehicle insurance and acceptable driving record. Background check and physical required.
Cedar Valley Services’ mission is to provide adolescents and adults who have vocationally-limiting disabilities with vocational training, habilitation, residential and supportive person served-driven services which will assist and enable them to attain their optimal level of functioning in daily living and work, as demonstrated in program outcome.
Job Type: Full-time
Pay: $19.00 - $23.00 per hour
