What are the responsibilities and job description for the Activities Coordinator, Memory Care position at Cedarbrook Senior Living?
AtCedarbrook Senior Living, we work hard every day to find out what "CelebratingLife Every Day!" means to our residents and their families, then we help themachieve these goals. We are seeking a dynamic Activities Coordinator for our Memory Care Neighborhood.
Responsibilities :
- Meetsregularly with Neighborhood Director to discuss format, purpose and progressfor individual and group programming for each level of care
- Personallyand through communication with staff members, evaluates the interest, abilitiesand needs of residents and plans appropriate activities based on these needsand available support
- Overseeslarge, small, and one-to-one events for the purpose of providing friendlyvisitations, sensory stimulation, reality orientation, and cognitivestimulation
- Independentlyadministers programs, classes, and off campus travel excursions as needed
- Administers the department's Interest Survey,MDS, Care Plan and Progress Notes in accordance with applicable codes
- AttendsCare Plan meetings and interacts with all disciplines of the health servicesteam, including nursing, social services, dietary, therapy and restorativeaides
- Effectivelycommunicates with applicable department heads, managers and employees on aregular basis to inform, exchange ideas, and coordinate services. Also shares insight, information, and reportsresponses and / or progress regarding residents
- Effectivelycommunicates with applicable departments regarding assistance needed in setup,implementation, participation, and need for use of common / shared space andservices
- Evaluatesrecreational programming on an ongoing basis through surveys and directinteraction with residents and families
- Arrangestransportation for off campus events
- Directsand coordinates volunteer screening and training, volunteer services andprograms for each level of care
- Administersactivities budget for the Memory Neighborhood including planning, purchasing,and maintaining necessary elements. Educates staff on budgeting and purchasing policies
- Recognizessigns and symptoms which signify a change in condition that require nursingintervention and reports these changes to the appropriate channels. Shares resident information with staff andfamily on a need to-know basis
- Attendstraining, meetings, and continuing education workshops (as approved) andcontinuously researches / keeps up to date on current recreational senior andlong-term care trends
- Helpspublish and distribute community newsletter and calendar(s) specific to memory support
- Dependableand able to meet schedule requirements (attendance and punctuality) with norestrictions
- Otherduties as assigned by Supervisor
