Must be a RN with MDS experience
Position Summary:
The MDS Nurse Coordinator is responsible for the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines.

Job Responsibilities
- Identify resident acuity and needs, help to determine specific care needs and communicate needs / expectations to families and responsible parties
- Complete assessments, PDPM and care plans for all residents assigned
- Monitor completion of PDPM's by other disciplines within time frames prescribed by regulatory guidelines
- Advise supervisor of incomplete and/or untimely assessments by disciplines other than nursing
- Ensure accurate, timely completion of the Triggers sheet for assigned residents
- Initiate care plans and supporting activities that will result in best possible outcome for assigned residents
- Monitor and ensure completion of 7-day look-back documentation by nursing staff within prescribed time frame
- Maintain current knowledge of PDPM assessment process
- Run the Restorative program in the community
- Responsible for Quality Measure Program
Qualification
- Licensed RN or LPN with the state of Illinois
- 2 years MDS and Care plan experience
- Knowledge of Medicare MDS and PDPM
- Experience in Restorative Programming, Fall care, monitoring and investigation prevention
- Knowledge of Medicare and Medicaid reimbursement
