Located in Echo Park, Los Angeles, Center for Better Life is seeking a bilingual (English/Korean) social worker who meets the below job qualification.
Community-Based Adult Services (CBAS, formerly known as adult day health care center)is a community-based day health program for older adults and adults with certain chronic medical, cognitive or mental health conditions, or disabilities who are at risk of needing institutional care to restore or maintain their optimal capacity for self-care and delay or prevent inappropriate or personally undesirable institutionalization, including;
- an individual assessment
- professional nursing services
- physical, occupational, and speech therapies
- mental health services
- therapeutic activities
- social services
- personal care
- a meal
- nutritional counseling
- transportation to and from the participant's residence and the CBAS center
QUALIFICATIONS:
1. Education
- Master’s degree in Social Work or equivalent education in the related field
2. Skills
- Bilingual in English and Korean
- Computer literate with basic skills in Microsoft Word and Excel
- Good oral/written communication skills
- Strong teamwork and collaboration skills
BASIC FUNCTION:
Under the direct supervision of the Program Director, responsible for the provision of CBAS (ADHC) social services, including assessment, care plan development, implementation of the care plan, and advocacy for each participant.
JOB RESPONSIBILITIES:
1. Perform initial screening for ADHC eligibility.
2. Perform psychosocial assessments. Contact family and other health providers as needed for histories. Obtain data related to the financial situation and social supports that are needed for initial enrollment.
3. In conjunction with the multidisciplinary team, develop individual treatment plans.
4. Perform psychotherapeutic interventions for participants and families.
5. Communicate regularly with family members, especially those in the high-stress risk of placement situations. Also, counsel families to involve them in the carryover of treatment plans at home.
6. Provide information and referral to participants and families. Act as liaison for them in dealing with public, private, and social service agencies.
7. Consult with the multidisciplinary team and develop discharge plans. Note in chart changes in these plans following staff conferences.
8. Do a home visit to assess the living environment, transportation, and accessibility to facilities, family relationships, support, and functional abilities when a new participant is enrolled.
9. Document appropriate participant and family contacts. Prepare quarterly progress notes and other reports as necessary.
10. Conduct participants reassessments every six months.
11. Develop and lead appropriate clinical and family support groups.
12. Other duties as assigned by Program Director.
