Come join our team in one of the largest Supports Coordination Organizations in Pennsylvania. CCR is currently seeking professional and self-motivated Supports Coordinators in the Allegheny County area to serve individuals diagnosed with developmental disabilities include Intellectual Disabilities and Autism.
Essential Functions:
1. Demonstrate knowledge and apply understanding of Supports Coordination services through the Office of Developmental Programs including the Bureau of Supports for Autism and Special Populations. Support individuals, families, and team members in navigating systems to support assessed needs of individuals living with a developmental disability.
2. Facilitate and participate in the development and periodic review of the person-centered Individual Support Plan (ISP). Ensure individuals are receiving the type, scope amount, and frequency of authorized services and supports approved in their ISP.
3. Monitor health and safety, complete incident management reporting as required by Program regulations as well as identify mitigation strategies to prevent reoccurrence.
4. Accurately document SC activities in services notes, track caseload needs including timeframes for required monitoring and ISP meetings, transition planning activities, high risk needs, etc.
5. Maintain utilization requirements set by program. Manage caseload with an average size of 35 participants.
Responsibilities:
Locate, coordinate, and monitor services for people with developmental disabilities in the community.
Travel throughout the region to monitor individuals in their home, day program, or residential setting. CCR provides mileage reimbursement at the state set rate!
Communicate effectively with direct Supervisor and co-workers. Be able to work as a team and facilitate meetings with individuals with disabilities, interdisciplinary team members including County Administrators to ensure supports and services are in line with assessed needs.
Daylight, Weekday hours, Flex Schedule, Opportunity to work remotely but in person visits and travel are required. No on call!
Qualifications:
Bachelor’s degree with successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social services. At least one-year experience working with individuals with disabilities preferred.
ACT 33/34 and FBI Clearances, valid driver’s license and proof of car insurance coverage must be provided prior to hire.
Must be able to work well independently without direct supervision and have excellent time management and organizational skills.
Must be comfortable using all forms of communication and be able to facilitate team meetings with individuals, families, and providers. Must be able to build rapport with individuals and families and be committed to be part of the team both in the field and internally.
