JOB TITLE: Bilingual Intake Coordinator-English/Mandarin Required, Family Support Program

DEPARTMENT:  Family Counseling

EXEMPTION: Non-Exempt


PURPOSE OF THE POSITION: This is an excellent position for someone experienced in providing direct services who wants to develop their leadership skills.  Master's level staff can earn clinical hours toward licensure. The Family Support Program is a preventive services program funded by the Administration for Children’s Services (ACS) providing counseling and case management services to families in Sunset Park and Southwest Brooklyn. The Intake Coordinator is a community facing position responsible for representing CFL’s mission and commitment to high quality, relationship centered, neighborhood-based services.

SUPERVISES: N/A


I. RESPONSIBILITIES

  • Responsible for managing referral and intake process for a preventive services program serving 96 families, collaborating with a Spanish-speaking Co-Intake Coordinator. 
  • Coordinate, create, and facilitate program outreach presentations and workshops for ACS and community partners. 
  • Cultivate and maintain partnerships within the organization and with community partners and schools to develop and maintain referral pathways.
  • Participate in community events to introduce the Family Support Program and other CFL programs.
  • Coordinate with the ACS referral team to screen referrals, conduct joint home visits with the ACS case manager, and complete intakes. 
  • Engage clients using a therapeutic, trauma-informed approach during the intake process.
  • Conduct intakes in office, in families’ homes and in community locations as needed. 
  • Assess families presenting concerns and appropriateness for program.
  • Utilize knowledge of the child welfare system to help clients understand their relationship to ACS, family court and preventive services, and to assess the appropriate level of services, as necessary.
  • Connect families to other community providers/resources if needed during the intake process.
  • Complete training in program's evidence-based practice model, Mobility Mentoring, and incorporate into intake process.
  • Complete all required intake documentation and data entry within deadlines.
  • Maintain tracking system for referrals and intakes.
  • Generate reports including caseload, cases opened and utilization.
  • Participate in weekly all-staff team meetings, and monthly supervisor team meetings.
  • Participate in biweekly intake review and planning meetings with the program director.
  • Carry a caseload of 3 families depending on the number of intakes per month.
  • Other duties as assigned.

II. QUALIFICATIONS

  • Master's degree in social work or a related field required. 
  • Bilingual: English/Mandarin required.
  • At least 1 year of experience working with immigrant families and accessing community resources.
  • Proficiency with Microsoft Office tools. Previous experience with CONNECTIONS and PROMIS databases preferred.
  • Excellent verbal and written communication skills.

IV. WORKING CONDITIONS

  • Share an office with one other staff member.
  • Access to an individual desk and computer.
  • Attend various meetings with agency staff, supervisors, ACS, community-based organizations.
  • Required to travel to conduct home and community visits.
  • Required to work some evening hours each week to accommodate working families.
  • Occasional weekend hours to attend community events.

V. SCOPE OF RESPONSIBILITY: 

The Intake Coordinator manages all aspects of the outreach, referral, and intake process for the Family Support Program, together with the Co-Intake Coordinator.  They are essential to developing effective relationships with our government and community-based partners through intentional engagement and communication. They are the ambassador for CFL, interacting at all times with a tone of respect and kindness, generosity and solution focused problem solving.

Salary Range:  $57,000-$60,000

Hybrid Schedule: Monday to Friday
