Position Summary:
Center For Family Services is seeking an experienced individual for the Patient Navigator position within its OORP program. Staff of the OORP program provide recovery support services for individuals treated as a result of an overdose. OORP services can only be initiated by hospital providers and services include support for individuals upon discharge from the hospital with follow up phone calls and supportive services, linking individuals to treatment and recovery services, and providing education to members of the community on overdose, treatment, and prevention.
Duties and Responsibilities:
- The Patient Navigator is a key member of the OORP team, that includes Peer Specialists as well as Case Managers, to help patients who have overdosed. The Patient Navigator has excellent verbal and written skills and effectively communicates with patients in the program. The goal of OORP is to help people stay engaged in recovery.
Qualifications
- Bachelor's Degree
- Five years' experience working with individuals with substance abuse disorders with minimum one year of experience with medication assisted treatment
- Federal and State regulations require employees in this position to be fully vaccinated. According to the CDC, fully vaccinated means to have received the initial vaccination(s) and at least one booster shot. As you apply for this position, you will be required to be fully vaccinated. - Current driver's license with a good driving record of at least three years
- Vehicle with current insurance and registration
