The Center for Human Development (CHD) is seeking an TBI Outreach Caseworker (Bilingual) to join our team! The TBI Outreach Caseworker is responsible for providing direct services and casework support to program participants with Traumatic Brain Injuries (TBI) who live independently in the greater Westfield/West Springfield and Springfield community.
Your responsibilities as a TBI Outreach Caseworker:
- Supporting program participants to achieve their individual goals and objectives, manage their ongoing health maintenance needs, and provide supports/skill building for all aspects of independent living, while providing opportunities to optimize their independence
- Help people to obtain and maintain all generic entitlements, responsibly manage their funds, and develop rewarding social and recreational experiences where desired
- Provide and maintain written documentation and assist Case Manager with Individual Service Plan development, assessments, and progress updates.
REQUIRMENTS:
- Bachelor’s Degree or equivalent experience
- Valid driver’s license and ability to provide transportation on an as needed basis
- 21 years of age or older
- Bilingual with proficiency in both English and Spanish required
SUCCESS FACTORS:
The ideal TBI Outreach Caseworker will require strong interpersonal skills, empathy for others, and a demonstration of commitment to serving those with significant needs. The ability to work well within a team model and independently will also make you successful. This role requires travel for home visits throughout Western Mass, including Hampden, Franklin and Hampshire counties with mileage reimbursement.
