What are the responsibilities and job description for the Caseworker position at CHD Brand?
The Caseworker provides outreach and supportive services to clients with psychiatric and/or substance use disorder diagnoses to enable them to utilize clinical treatment services and other supports. The Caseworker assists clients to attain their clinical treatment plan goals in outpatient and other levels of care and works to mitigate barriers to success.
Responsibilities include:
- Provide service coordination and linkages to the community
- Provides outreach services to clients, delivering mobile services in a number of community or institutional settings including inpatient units, homes, day programs, etc.
- Assist clients in obtaining benefits, housing and health care as needed
- Encourages and facilitates utilization of clients’ natural support systems as well as recovery-oriented, peer support and self-help supports and services
Minimum Requirements:
• Bachelor’s degree from an accredited educational institution or at least three years of relevant work or lived experience that is acceptable by MBHP waiver • Ability to pass certification for CPR, First Aid • Valid driver’s license; reliable transportation for business use • Ability to informally counsel and support clients with psychiatric and/or substance use disorder diagnoses
