What are the responsibilities and job description for the Caseworker position at CHD Brand?
The Center for Human Development, (CHD) is seeking a Permanent Supportive Housing (PSH) Caseworker to join our Diversion, Shelter and Housing (DSH) team. The PSH Caseworker strives to meet overall family goals and provides support to program families that will assist in rehousing.
Your responsibilities as a Caseworker:
The PSH Caseworker will maintain a high level of commitment to program families. While the Family Case Worker’s primary focus is to assist families in the EA shelter system with accessing permanent housing, s/he also (1) provides a supportive relationship to clients in need of services that address mental health, substance abuse, and other collateral issues and (2) works with families and staff to support a safe, positive, and comfortable shelter environment that minimizes the trauma of family homelessness.
REQUIREMENTS:
- Multiple years of experience in case management and social services preferred
- High school diploma or GED required
- Reliable, registered & insured vehicle and approved driving record check
SUCCESS FACTORS:
Caseworkers develop partnership with families monthly Rehousing Plans that identify goals and tasks that will facilitate housing access as rapidly as possible. Link families with publicly-funded rehousing benefits (e.g., HomeBASE, relocation) and other forms of rental assistance in order to supplement earnings, savings, and other forms of income. They interact with family following the Motivational Interviewing model in order to assess needs, provide information, discuss problems, set limits, and encourage the family’s growth and development.
