Essential Duties and Responsibilities:
Duties may vary based on specific program and position, and may include but are not limited to the following:
- Assist individuals in identifying and articulating their needs for services and supports.
- Coordinate, negotiate, and facilitate a comprehensive array of services and supports to address identified goals and desired outcomes.
- Provide behavioral intervention services, training, and support tailored to the needs of the assigned caseload.
- Ensure that planned services and supports are effectively implemented and properly documented.
- Connect individuals with appropriate community resources, services, and supports as needed.
- Maintain accurate and timely documentation of all meetings, progress notes, and activities related to individual service plans.

Qualifications
Education and Experience:
- Bachelor’s or Master’s degree in a Human Services-related field such as:
- Psychology
- Sociology
- Social Work
- Rehabilitation
- Counseling
- Human Growth and Development
- Gerontology
- Special Education
- Educational Psychology
- Early Childhood Education
- Early Childhood Intervention
- Licensed Registered Nurses are also eligible for this position
Certifications, Licenses, and Registrations:
- Valid Driver’s License required
