General Description:
Under the general supervision of the Director of Adult Mental Health or designee, the Case Manager will work primarily with individuals under the Community Supervision and Corrections Department (CSCD) program (probation), as well as those with bond conditions requiring mental health services. It will also assist with individuals referred from parole. This position is responsible for assessing client needs, facilitating service linkage, coordinating with probation and parole officers, and ensuring that individuals receive appropriate mental health and supportive services. This role will develop and implement recovery plans, monitoring compliance with treatment and legal requirements, and advocating for clients within the criminal justice system. The Case Manager will collaborate with law enforcement, courts, and community partners to improve outcomes for justice-involved individuals. Duties may involve data collection, case tracking, and networking with stakeholders to enhance service accessibility. While work is primarily conducted in outpatient settings, some jail-based visits may be required. The position requires direct contact with clients, families, and service providers. The incumbent must be able to lift more than 45 pounds and perform job functions that involve sitting, walking, lifting, and bending. A flexible schedule is required, and a company vehicle will be provided for job-related travel.
Qualifications:
A bachelor’s or master’s degree from an accredited college or university in psychology, social services, or a related field is required, meeting qualifications for a Qualified Mental Health Professional (QMHP). A minimum of six (6) months of experience in the behavioral health field is strongly preferred. The candidate must possess knowledge of mental illness, substance use disorders, intellectual disabilities, medications, and the criminal justice system. Professionalism in working with court officials, probation/parole officers, law enforcement, and community agencies is essential. Strong oral and written communication skills, along with proficiency in computer use and documentation, are required. The candidate must be able to work in high-pressure situations and manage crisis intervention when needed. Travel within a seven-county service area is required during shifts, and a valid Texas Driver’s License with a clean driving record per CFLR requirements is mandatory. Additionally, the candidate must pass required background checks, prescreen evaluations, and staff development training.
