CLW is a consumer-directed, multi-programmed, multifaceted agency, dedicated to the empowerment of all persons with disabilities to live as independently as possible. CLW provides five core services: Advocacy, Skills training, Peer support, Information and Referral and Transitions.
Responsibilities:
IL-LTSS Responsibilities
- Conduct face-to-face visits with consumers, as requested by Enrollees or Plans.
- Participate in assessments and develop care plans to ensure all relevant consumer needs are addressed in a fully integrated manner.
- Provide ongoing monitoring and support to ensure the continued success of care plans.
- Conduct LTSS assessments and monitor LTSS delivery.
NHI/OC Responsibilities
- Assist with applications for Home and Community-Based Services (HCBS) waivers when applicable.
- Coordinate transitions from facility-based to community-based settings, including:
- Housing search assistance
- Coordination and purchasing of transitional assistance goods and services
- Post-transition follow-up and support
- Provide Options Counseling and educate consumers/guardians on available services and supports.
General Responsibilities
- Travel within and beyond the service area as needed to meet with consumers or attend events/meetings.
- Maintain accurate and timely data entry in multiple systems to record, collect, and update clinical and demographic information.
- Assist the IL Department with the completion of required monthly and quarterly reports.
- Ensure strict confidentiality of consumer information.
- Other duties as assigned.
Required Skills and Qualifications
- High school diploma or equivalent (GED) required.
- Minimum 1 to 2 years of related experience working with community resources and local service systems for persons with disabilities or older adults.
Experience with:
- Discharge/transition planning
- Excel, Microsoft Office, and database management
- Information and Referral services
- Teaching Independent Living skills and providing advocacy while promoting self-empowerment
- Strong knowledge of Independent Living issues and philosophy.
Familiarity with:
- MassHealth, state agencies, and provider service systems.
- State and federal housing regulations.
- Civil rights for individuals with disabilities.
- Ability to:
- Work independently and as part of a team.
- Work effectively with individuals with diverse disabilities and cultures.
- Prioritize and meet deadlines.
- Provide informed advocacy and guidance.
- Write individualized care plans and communicate effectively across service systems.
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Remain in a stationary position for extended periods of time.
- Must be able to lift up to 15 pounds at times.
