Adult Case Manager
Full-Time/Non-Exempt/Great Falls
Position Overview: A paraprofessional with knowledge and skills to effectively perform case management duties to the seriously mentally ill who are evaluated to meet both financial and clinical criteria
Job Duties:
- Maintain caseload of assigned consumers, 24 billable hours weekly
- Complete comprehensive strength-based assessment and reassessment every 90 annually or as needed
- Develop written treatment plan that coordinates with other services including family supports and outside community services every 90 days
- Develop crisis plan based on recommendation of the clinician
- Monitor progress/eligibility
- Helps client obtain appropriate entitlements/advocate
- May develop budget plans with client & work with contracted payee representative
- Must spend 50% of time out of office in client homes or in the community
- Coordinate services
- May perform duties of ATA as required
- Complete and appropriately maintain all required paperwork in a timely manner
- May need to transport clients in private vehicle
Minimum Qualifications: Bachelor’s degree in human services field, plus as least one year of full-time experience serving people with mental illnesses (serving SDMI adults is preferred) or individuals with other educational backgrounds who, as providers, consumers, or advocates of mental health services have developed the necessary knowledge and skills (graduation from high school or GED and a combination of relevant experience and/or education equaling at least four years.
$2,000.00 Full-Time Sign on Bonus
Hourly Wage = $15.55 DOE
