Position Overview: A paraprofessional with knowledge and skills to effectively perform case management duties to the seriously mentally ill who are evaluated to meet both financial and clinical criteria.
Job Duties:
- Helping adults with severe disabling mental illness by assisting the client to obtain services, such as income support, transportation, housing, rehabilitation services, assist with treatment plans, prevocational and vocational services.
- Responsible for planning and coordinating care and services to meet individual needs of seriously emotionally disturbed Youth.
- Assists the Youth in obtaining necessary medical, social, psychological, nutritional education, vocational training, supported employment, and other services directed in part toward the prevention of out-of-community placement of the Youth or the Youth’s return from acute or residential psychiatric care, and when appropriate the preservation of the Youth’s family unit.
- Maintain caseload of assigned clients and meet productivity goals.
- May need to transport clients or family members in company vehicle when available.
Minimum Qualifications: Minimum qualifications are a bachelor’s degree in a human services field and at least one year of full-time experience serving people with severe disabling mental illnesses. Other education and experience may be considered. Such as a combination of relevant experience and/or education equaling five years. Travel required. Must have vehicle, valid driver’s license, and current vehicle insurance to transport clients.
$500.00 Part-Time Sign on Bonus
Hourly wage: $15.55 DOE
Many Rivers positions require a Criminal and Motor Vehicle background check as well as a background check with DPHHS. Will be required to take Sexual Abuse Training.
