Quality Care TCM Coordinator

Summary: The Quality Care TOC Coordinator is responsible for facilitating transitional/post-acute care for patients by scheduling and assisting in a timely, high quality, and efficient manner. The coordinator will work directly with our patient population and their families, physicians, clinicians and other medical personnel.

Education: High School Diploma. Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) licensure required.


Experience:
Minimum 2 years of recent clinical experience.

Essential Job Functions:

  • Supports quality improvement by guiding patients as they transition from an inpatient facility, such as a hospital, rehab, or short-term nursing home to their home, domiciliary or assisted living facility.
  • Reviewing and monitoring admission and discharge reports from hospital and payer portals and reports.
  • Taking incoming calls from area hospitals and scheduling patients for follow-up visits.
  • Conducting patient outreach within two days of discharge to address patient status as well as coordinating a follow-up visit for patient within 14 days of discharge.
  • Ensuring that all documents related to patient’s discharge have been obtained and scanned into chart prior to visit.
  • Communicates with the physicians when needed, regarding patient concerns or with other matters as they arise.
  • Monitors key internal and external clinical quality metrics reports, and other performance reports.
  • Collaborates with practices to identify opportunities for improvement and develops supportive plans to target opportunities using quality improvement methods and tools.
  • Communicates with manager regarding physician inquiries, requests, and/or concerns.
  • Assists in providing education for staff and providers on TCM initiatives to maintain or improve performance.
  • Provides support for the practice to meet organizational TCM goals through optimizing provider and patient engagement.
  • Attend planned meetings scheduled by the Manager of Quality Care Coordination.

Performance Requirements:

Knowledge, Skills and Abilities

  • Strong working medical knowledge of the clinical needs and disease processes for the chronically ill population
  • Knowledge of general health care practices, office workflows, and medical terminology
  • Excellent telephone, oral and written communication skills.
  • Ability to work autonomously and be directly accountable for assignments
  • Strong computer skills, including Microsoft office and EMR experience


Equipment Operated:
Standard office equipment including computers, fax machines, copiers, printers, telephone, calculators, etc.


Work Environment:
Position is primarily in a well-lit office environment. This position may require limited local travel throughout the business day.


Mental/Physical Requirements:
While performing the duties of this job, the employee is regularly required to talk or hear. The employee is primarily sitting at a desk utilizing a computer.
