CenterCare Hospice is looking to add a Registered Nurse for Admissions and Case Management to their team. Our service area covers Colorado Springs, Pueblo and Canon City.

RN- Job Summary: Provide nursing care in the patients place of residency in accordance with current federal, state, and local standards, guidelines, and regulations that govern our organization, and as may be required by the Director of Nursing or designee.
RN- Essential Job Functions:
Admissions process beginning to end
Administer services within the applicable scope of nursing practice
Determine the scope and frequency of services needed based on the acuity of the patient and the family/caregiver needs
Assess the ability of the caregiver to meet the patients needs upon admission and throughout care
Communicate with physicians, care team, patients and family members effectively
Consult with and educate the patient and family/caregiver regarding the disease process, self-care techniques and end of life care
Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician, and in accordance with established procedures
Follow drug administration protocol, to include the disposal of drugs and narcotics as required, and in accordance with established procedures
Perform charting duties as required and in accordance with established charting and documentation policies and procedures and applicable state and federal regulations
Review care plans to verify that appropriate care is being rendered. Communicate to the care team
- Graduate of an accredited school of nursing
- Current state licensure as a Registered Nurse
- Two years experience as a Registered Nurse in a home care setting
- Must have experience with new admissions and its process
- Current CPR card and Hepatitis consent/declination
- Valid state drivers license and reliable automobile
- Proof of current automobile insurance
