We are seeking an exceptional Registered Nurse Case Manager to join our La Junta team. As a key member of our team, you will play a vital role in coordinating patient-centered care plans that prioritize comfort, dignity, and exceptional care.
Responsibilities
- Lead the coordination of compassionate, patient-centered care plans

Requirements
- Active Colorado Nursing License
- 1 year of home health or hospice experience preferred
- Strong organizational and communication skills
