Job Description
What You’ll Do:
Leadership & Supervision
- Directly supervise and mentor LVNs/LPNs/RNs in the field according to state guidelines
- Provide clinical oversight, training, and ongoing support to homecare staff
- Conduct performance evaluations and competency assessments
Care Coordination & Clinical Oversight
- Coordinate and oversee the implementation of physician-directed Plans of Care
- Complete admissions, recertifications, resumptions of care, and all required clinical documentation
- Provide patient and caregiver education to promote health and wellness
- Monitor for changes in condition and communicate effectively with interdisciplinary teams
Compliance & Quality Assurance
- Ensure care delivery complies with MGA, state, and federal regulations, as well as accreditation standards
- Review documentation for completeness, accuracy, and reimbursement compliance
- Participate in quality improvement initiatives, risk management, and reporting
Collaboration & Program Development
- Partner with Client Services Managers and the Director of Clinical Services to enhance client satisfaction and clinical outcomes
- Participate in strategic initiatives to support operational growth and patient care excellence
Qualifications
What You’ll Bring:
- Active RN license in the state of practice (BSN preferred)
- Minimum 2 years of clinical nursing experience (Home Health or Acute Care preferred)
- Demonstrated leadership and supervisory skills
- Strong communication, organization, and critical thinking abilities
- Familiarity with clinical documentation requirements and compliance standards
- Current CPR certification, valid driver’s license, and reliable transportation
Additional Information
Benefits are available to eligible employees on the first of the month following 30 days of employment and include:
- Base Salary $68,000-75,000
- $50/mo cell allowance and $300/mo auto allowance. ($4200 annually)
