Patient Coordinator

This full time and/or part time, Patient Coordinator position at Centerpoint Health In Franklin Ohio, supports the mission and vision of Centerpoint Health by working directly with patients to help them navigate and access community services while adopting healthy behaviors. They will promote, maintain, and improve individual and community health through screenings and data monitoring. This position is grant funded and depending on future funding will determine the longevity of the position.

Patient Coordinators provide creative strategies, materials, and tools for training, educating, and changing lifestyle behaviors and are active promoters of health in their community.
Job Duties:
· Screen all incoming patients including eligible Medicare and Medicaid beneficiaries in the designated primary care clinics.

· Help individuals, families, groups, and communities develop their understanding and access to resources including health insurance, food, housing, quality care, and healthcare information.

· Help to build understanding and social capital to support healthier behaviors and lifestyle choices.

· Hold or staff a table at workshops, group discussions, and community events to learn/advocate about ways the community can promote good health.

· Arrange transportation for healthcare appointments.

· Advocate for patient and community health needs.

· Provide culturally appropriate health education and information.

· Provide informal counseling and guidance on health behaviors.

· Other duties as assigned.

Employees working under the Health Collaboration grant will be assigned these additional duties:

· Participate in planning and execution of program.

· Screen eligible Medicare and Medicaid beneficiaries in the designated primary care clinics

· Attend screening, navigation, and cultural sensitivity training, as applicable.

· Participate in quarterly AHC Advisory Council meetings.

· Participate in QI advisory committee, help inform annual gap analysis, and create associated process improvement plan.

· Providing dedicated staff to conduct screening

· Identify and refer high risk community dwelling beneficiaries to community navigation services focused on health-related social needs including housing, utility, food, transportation, and interpersonal violence.

· Track and submit required AHC data elements in Healthify

· Participate in project learning forums.

· Ensure screeners are culturally competent and knowledgeable in community resources.

· Measure and report program operating costs and total number of beneficiaries served.
Certifications/Credentials:
· High school diploma

· Associates degree preferred

· Community Health Worker certification preferred
Additional Skills/Qualifications:
· Able to spend significant time standing and walking.

· Able to use equipment required for the delivery of services.

· Detail-oriented

· Possess strong working knowledge of data collection. Familiarity with statistics preferred.

· Ability to work collaboratively within an integrated health care environment.

· Demonstrate strong oral and written communication skills when interacting with patients, families, and other health care providers.

· Ability to work with interruptions, manage priorities, and work unsupervised.

· Ability to be adaptive, flexible, relatable, and responsive to the needs of the health center and the needs of patients.

· Proficient computer skills, including Microsoft Office. Previous knowledge of Electronic Medical Records preferred.

Bloodborne Pathogens Exposure: Category I

*Will need to spend significant time standing and have the ability to use equipment required for the delivery of services.

*This position is contingent upon a successful background check and drug test.
