THE JOB
As a Care Coordinator, you will provide targeted support to individuals receiving services, assisting them and their families in navigating community resources effectively. This role involves managing care transitions, enhancing engagement in services, and ensuring that coordinated systems of care address assessed needs in a timely and comprehensive manner. Key responsibilities include conducting assessments, obtaining consents and medical records, and collaborating with internal and external programs to link individuals and families to healthcare providers and resources. The position requires strong organizational skills to manage follow-ups, schedule appointments, and provide outreach, with occasional local and out-of-town travel to meet community needs.
The position is full-time, Monday through Friday. Benefits are included with full-time employment.

WHAT YOU HAVE
Required:
- High school diploma or equivalent
- Bachelor's Degree in Social Work, Health Promotion, or other health-related field OR valid LPN, RN, or LMHP/LIMHP license in the State of NE
- Experience working with people from diverse backgrounds, cultures, lifestyles, etc.
- Knowledge of community resources, alternative and traditional health perspectives
