What are the responsibilities and job description for the Patient Care Coordinator position at Primary Care Partners?
Patient care coordinators act as liaisons between patients, the physician, and any other point of contact needed to ensure that patients receive the care they need. They communicate with patients to ensure they understand their medical conditions, medications and any other instructions given. As a patient care coordinator, you will be working with a variety of outside entities to coordinate patient’s needs.
As a patient care coordinator, you are a major part of the care team. Assessing and screening patients during Annual Wellness Visits as well as providing education and referrals to any screenings that may be needed. You will create care plans including outcome goals and follow up with patients to measure progress. Patient care coordinators will follow up with patients who have had a hospitalization or visit to the emergency department.
· Plan, coordinate and carry out patient operations
· Perform Annual Wellness Visit for Medicare patients
· Manage chronic care patients
· Follow up on patients seen in the emergency room or discharged from the hospital
· Follow up on patients discharged from Rehab, SNF or other inpatient facilities
· Communicate and educate patients with certain new medical diagnosis
· Develop care plans
· Risk stratify patients
· Connect patients to community resources
· Increase utilization of preventive care
· Patient education
· Increase patient’s ability to self-manage conditions
· Medication reconciliation
· Reduce emergency room visits and hospital readmissions
· Communicate with family members
· Other duties and responsibilities as assigned
Experience/Knowledge:
· LPN or RN
· Minimum of five years’ experience in nursing
· Proficiency in Windows applications (Word, Excel, PowerPoint, Access) and healthcare software
· Strong organizational skills
· Effective customer service skills
· Skill in writing policies and procedures
· Professional communication and presentation skills, including face to face, email, telephone, and video conference.
