Centers Dialysis Care is seeking a dedicated Social Worker to join our dialysis center in New York. We are a high-level network of dialysis facilities located in Brooklyn, Bronx, and Rockland County.

Centers Health Dialysis operates five centers throughout the Tri-State Area, providing quality care for over 13 years to our dialysis community. We focus on delivering an exceptional patient care experience in outpatient dialysis facilities, both onsite and offsite of skilled nursing facilities. As an independently owned and locally managed organization, we offer individualized care often missing from larger organizations.

Duties:
- The Renal Social Worker uses clinical skills in rendering treatment and referral services to all End Stage Renal Disease patients and their families, with the goal of improving patient care and meeting therapeutic objectives.
- Conducts psychosocial assessments and documents patient status on a quarterly basis and as needed.
- Actively participates in patient care plan meetings and interprets facility policies and procedures to patients and families when necessary.
- Participates in in-service training within the facility and compiles educational material to aid in psychosocial education.
- Initiates referrals to community agencies for follow-up psychosocial care and approaches patients, families, and visitors with courtesy and respect.
- Maintains confidentiality and privacy of all patients, reducing anxieties through clear explanations of procedures and delays.
- This position requires a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) licensed by the State of New York.
- Holds a master's degree in social work from a school of social work accredited by the Council on Social Work Education.
- One year of dialysis experience preferred but willing to train the right candidate.
