Job Description:
Overview: As a Home Health Speech-Language Pathologist, you will evaluate, direct, and provide speech/language pathology services to patients in their homes or facilities. You will participate in developing and reviewing treatment plans, utilize professional skills and judgment to assess and treat disorders, administer diagnostic tests, and observe patient changes and report them to supervisors and physicians.

- Participate in developing and reviewing treatment plans with healthcare teams.
- Utilize professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing.
- Administer diagnostic tests and interpret results to inform treatment decisions.
- Observe and record patient changes and progress, reporting them to supervisors and physicians as needed.
- Provide instruction and training to patients and families on using alternative communication systems when appropriate.
- Maintain accurate documentation of patient care and submit reports as required by the company.
- Collaborate with healthcare teams to ensure seamless care coordination and discharge planning.
- Protect confidential patient information and maintain the highest standards of professional conduct.
- Attend internal staff development programs and obtain continuing education as required by company policy.
