What are the responsibilities and job description for the Home Health RN Case Manager position at CenterWell Home Health?
At CenterWell Home Health, we are dedicated to providing personalized care to patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization. As a skilled nursing professional, you will play a vital role in our care teams, working alongside nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers to help patients rehabilitate, recover, and regain their independence.
The ideal candidate for this position will have a strong background in med-surg, ICU, ER, and acute experience, with a minimum of one year of nursing experience preferred. You will be responsible for administering on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager. This includes conferring with physicians in developing the initial plan of treatment based on physician's orders and initial patient assessment, as well as revising the plan in consultation with physicians based on ongoing assessments and as required by policy/regulation.
You will also coordinate appropriate care, encompassing various healthcare personnel, report patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis, and implement patient care plans in conjunction with patients and families to assist them in achieving optimal resolution of needs/problems. Additionally, you will discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
