Home Health RN Case Manager

FLEXIBLE SCHEDULING WITH NO WEEKEND REQUIREMENT

We are currently looking for a Home Health Case Manager RN that is interested in joining our team for Jackson county and surrounding areas.

Job Requirements and Responsibilities:

• Conducts in-person patient interviews and physical assessments
• Assures quality of care by measuring health outcomes against patient care goals
• Initiates and implements the plan of care and ongoing recertifications
• Communicates patient conditions and collaborates with appropriate providers to deliver care when patient needs evolve
• Initiates the plan of care and makes necessary revisions
• Provides education to patients and families on proper home health care procedures
• Responsible for the completion of all home health documentation, including admissions, recerts, discharges, and all OASIS documentation
• Conducts supervisory visits when necessary to monitor home health aide performance
• Work to decrease readmissions by promoting preventative care and ensuring continuity of care
• Serve as case manager for Medicaid patients.

Qualifications/Skills:

• Registered Nurse (RN) License, required
• One (1) year of professional experience practicing as a Registered Nurse (RN) in home health or similar setting; previous case management experience preferred
• Medicaid Home Health, preferred

Job Type: Full-time

Pay: From $42.00 per hour
