Job Title: Home Health RN Case Manager
About the Role:
You will be a key member of our care team, working closely with patients and their families to provide high-quality home health care services. Your responsibilities will include providing admission, case management, and follow-up skilled nursing visits for home health patients, administering on-going care and case management, and coordinating appropriate care with various healthcare personnel.

The estimated salary for this position is $65,000 - $95,000 per year, based on full-time (40 hours per week) employment at the time of posting.
